Community Mental Health Case Worker

Position Summary
The Community Mental Health Case Worker will provide individualized support to residents of Eden Village, focusing on mental health advocacy, coordination of care, crisis intervention, substance (Opioid) abuse recovery, and emotional wellness. This role is vital in fostering long-term stability, recovery, and integration for formerly unhoused individuals.
Key Responsibilities
- Carry a weekly client case load of 32 Eden Village residents
- Conduct mental health assessments and ongoing evaluations of resident needs
- Develop, implement, and monitor individualized service and (Opioid) recovery plans
- Host weekly onsite sobriety (Opioid) recovery groups
- Administer drug tests with Eden Village neighbors
- Provide client-centered case management, including referrals to mental health professionals, substance (Opioid) abuse treatments, primary care, and social services
- Offer crisis intervention and de-escalation support as needed
- Assist residents in navigating community resources and accessing benefits such as Medicaid, Social Security, and Disability
- Maintain accurate and timely documentation in compliance with HIPAA and internal policies
- Facilitate life skills, wellness, peer support groups, and sobriety (Opioid) groups as appropriate
- Collaborate with interdisciplinary teams, including medical, mental health, physical, social, vocational, and spiritual care
- Serve as a consistent, compassionate presence to promote trust and community healing
- Other duties as deemed necessary
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, OR 2+ years of experience in mental health, homelessness, or related social services field
- Knowledge of trauma-informed care and housing principles
- Ability to work effectively with individuals from diverse backgrounds, including those with co-occurring disorders
- Strong communication, organizational, and crisis intervention skills
- Valid driver’s license and reliable transportation required
- Certification or licensure (e.g., LCSW, LPC, Case Manager Certification) is a plus
Working Conditions
- Work is primarily on-site at the Eden Village campus
- Some local travel for outreach or appointments may be required
- Flexible schedule may include occasional evenings or weekends
